A mother of two was killed after drinking a cocktail of drinks and drugs and driving the wrong way on a highway for two miles.
38-year-old Georgina Coombs bumped into an innocent driver after rowing and storming with guests at a house party.
It entered oncoming traffic on the M4 before plowing into driver Dale Jones, who had sustained life-changing injuries.
The investigation in Newport, South Wales, found that Ms. Coombs was two and a half times over the alcohol limit and had used cocaine.

Gwent Coroner Caroline Saunders said: "Her partner followed her, but Georgina had already left the house in her Vauxhall Astra when he came outside."

The investigation found that the police had planned Ms. Coombs' trip through eyewitnesses, video surveillance and dash cam footage.
The coroner said, “Georgina went on the M4's western lane, but she's going in the wrong direction against the flow of traffic.
Georgina drove 2.8 km on the M4. She comes across several cars that get out of the way and whose lights flash.
It seems that Georgina tried at some point to avoid oncoming traffic.
"It's hard to understand why she didn't recognize her mistake and pulled her hard shoulder."
The coroner said Ms. Coombs, of Cwmbran, South Wales, had consumed "a significant amount of alcohol" before crashing into Mr. Jones' black BMW on February 22 last year.

The coroner added: “Georgina had used cocaine before she died.
"I think the high alcohol content was the reason Georgina drove in this dangerous and deadly way."
Her family said in a statement at the time: “Georgina was involved in a tragic accident that left two children and a big hole in the family that were really devastated.
"She'll really be missed by everyone who knew her."
